October 15th Meeting Notes
Reconvened to specifically discuss event time period
- Reviewed our discussion from September 1st meeting by detailing our proposed changes to the position statement
- Updating CP-CRE to CPO to include more relevant organisms - future meeting to discuss "core" surveillance organisms and how to write that out
- Adding WGS to laboratory criteria
- Updating event time period to be standard across clinical/screening cases with potential of elongating event time period
- Discussion around how frequently states saw same organism/CP gene over more than a year
- Seemed like this was somewhat common in New England but less common elsewhere
- Long tenured CPOs were more closely related to comorbidities than specific gene
- Discussion around how submission criteria to MuGSI (every 30 days) can create different event time periods in different surveillance systems
- A larger question around the purpose of surveillance developed again - there seems to be a trade off between a) a short event time period allowing frequent capture of data (which helps quantify the burden in states to some degree) with b) counting a person once per organism/CP gene combo which is both easier to report and helps the case counts reflect new, spreading disease.
- Both have informatic consequences - either updating the MMG to allow update of cases reported previously with new info if cases are only counted once or having to work analytically to describe new cases if case count doesn't reflect spread of disease (which to be fair - would have to happen with the current case definition).
- Ultimately decided that while having a standard time line for screening and clinical cases makes sense, we wanted to look at more data around long tenured CP-CRE and colonized individuals to empirically determine a time line that might make sense.
- MuGSI representative (Jennifer Bulens) said that their data (going back to 2011) would be good to look at
- States with examples will also check their surveillance systems and report back
- Re-emphasis of our stated goals:
- Future proofing position statement
- Adding notifiability to make things easier for jurisdictions
- Not adding reportingburden
- Next steps:
- Meeting in early November to discuss "core" organism surveillance issue
- Nov. 16 HAI Subcommittee to discuss progress
- Division of Position Statement labor after that
