Consent form for release of hospital discharge data

Our IRB has received several applications from investigators performing clinical trials related to social determinants of health (eg provision of housing), where subjects will consent (or not) to release of administrative data from public health  (eg hospital discharge data)  and other state agency databases (eg correctional) to the researchers.    We believe the part of the consent for release of health dept. info should be separate, and that subjects should understand that HIV,  substance use, and mental health related diagnoses may be included in hospital discharge data. 

1)Do any health departments have a consent form for release of  identifiable hospital  discharge information ?
2)If so, does your consent form treat certain information (eg HIV diagnosis) differently in the consent form?                                                                                                                           3)If the IRB requires that sensitive information be treated differently ( eg research participants may choose to allow release of all discharge diagnosis information except substance use diagnoses) is  your health department  able to efficiently delete  such information from  a large line list (eg thousands of patients) with up to 26 discharge diagnoses per patient?
