Hi all - What date(s) do you use for informed consent forms for expedited studies that have been transitioned under the Revised Common Rule? Do you still use an expiration date?

Hi Alysia - Our IRB does not include dates on the consent form, but documents them as approved along with the protocol and all study documentation in our approval letters. We require all study documents, including the protocol and consent forms, to have a version date assigned. So, if amended, the version is easy to update and keep track of.
