Since we are in a high incidence state, we collect data on LD cases almost exclusively through faxed forms. We were having trouble getting information on "physician diagnosis" for probable cases and were having difficulty getting onset dates for certain symptoms, which didn't allow us to evaluate if IgM testing was valid. Attached is our new form, which seems to have improved the local health department's ability to get this information. Sharing if helpful.
