📈 Codes for AOE COVID-19

Hi,
while we are also developing the 'universal' csv, in the meantime I'm sure a lot of us are encountering shifting in tests away from the traditional provider ordered to walk-on testing. Some of those sites have screening questions. Does anyone have a handy code set or reference list they've developed for those questions (symptoms, underlying conditions) they'd be willing to share? Thanks!
