COVID-19 Laboratories in other states

Hi All-
With COVID-19 testing, we are seeing a lot of new laboratories running tests for our state that are not sending via ELR.  Is anyone getting ELR from these laboratories and would you be willing to share your contacts?  You can also email me directly at tania.bechtel@la.gov

Arkansas is getting Viracor as well as AEL electronically. We had to come up with an unconventional solution though, due to the fact that Viracor nor AEL are capable of creating or sending HL7 messages.

We used our HIE (OHIT/SHARE) to create an electronic feed –sFTP I believe – where Viracor and AEL send csv files with the most basic and pertinent information. Our HIE’s programmers have developed an application that takes these csv files and makes an array of each row, iterating over each one and creating HL7s out of them. Our HIE then sends this to us over a SOAP connection to our Rhapsody which ingests them.

It took us a couple of weeks to get them clearing validation and ensuring the data was complete but we’re happy to report getting results electronically now.

I have both Viracor and AEL continuing to send faxes so my team can validate the ELR exists for every fax received.

Complicated, I know – but these are complicated times that require out-of-the-box thinking to solve real world problems.

Michael, can you share the CSV format they are using? We're trying to develop the same type of delivery method, but I'm going to have to convert the CSVs to HL7 myself. I'm trying to nail down a CSV format that will have all the info we need.

Hi Jason,
Unfortunately I do not. Since it is our HIE that receives these csv files and converts into HL7s—sending to us…all I see is the HL7 messages. I spearheaded the project, worked with their developers, and validated structure/content though. I can reach out to our HIE and have them provide some sort of data dictionary, or a template at the least of how they are receiving this. Will take me a bit of time but I’ll get to you ASAP.

Hello,

TN is converting tab-delimited files into HL7 messages. We are offering this option for labs that are not onboarded for ELR and perform a high volume of COVID testing for TN residents.

The attached file contains the initial two routes that we use to convert the tab delimited spreadsheet files to HL7. In the first route, “0A – CloverleafImport”, there is a java script filter named “Is this file Pathgroup, Solstas, SS or PTBMIS?”. This is where we distinguish the different partners, using the tab delimited header (first line in the file). The second route, “0B - PG and Solstas”, is a bit more complicated, but the different mappers are located within that handle the conversion.

I have also attached the template we share with our labs. We recently included 5 additional auxiliary fields as placeholders (AUX_1 to AUX_5) just in case we want to add more columns to the spreadsheet after moving it to production. Hope this helps!

VA is requesting a pipe delimited flat file. I have attached our file requirements. We are still in the process of developing an HL7 message based off of this.

The routes in that RLC won't load, the comms points weren't included in the save. The definitions and maps did load though.

Hey Jason,

Our Rhapsody programmer said you should only need to load the routes and then add your comm points. Our comm points that are used consist of the directory pickup and a sink. Our directory comm points would only work if you are setup to pick up from the same location that our Rhapsody does.

At least in Rhapsody 6.2, the RLC won’t load the routes without their comms points. Even if the location is different, the comms points will load if they’re in the save, they just won’t turn on, which is fine.

Hi Corrine -- we just heard from ViaCor -- they are doing so few tests for CT that we are going to deal with just faxes. However, if someone gets a csv/pipe delimited file from them, let me know, maybe we (CT) can piggyback on that. We are building a non-conventional method for flat files that aren't in a universal format (we have developed our in given COVID-19 testing) so either format would work for us for this tiny volume lab.
