COVID reporting Facility Type

Hi all,
on the CELR side we are creating a categorization of performing facilities / ordering facilities and reporting facilities to facility types (hospital, public health, national etc) but running into issues, when the CLIA / NPI numbers are NOT sent to CELR.
Do the jurisidictions have a category list for their submitters and if so, is that sharable?

For PA, I use the following types in our tracking sheet:

Education (any school of any level typically using POC machines)
POC (any entity using POC machine which is not another 'type', can include mobile/pop-up labs)
Hospital
Reference (any lab which is not a hospital)
Local-DOH
State-DOH
At-Home
Sita Smith
We separate out Higher Education (Colleges) and schools.
We also try to identify LTCFs/SNFs

Thanks you for listing the categories - can you share the actual mapping (name (or ID) of the facility that will be in your CELR file to the category) with the CELR TA folks by chance?

That would be a messy DB extract of how our NEDSS system uses a “Trading Partner Profile”. We have to create user accounts in all environments of PA-NEDSS for ELR users, just like they’re a normal data-entry reporter. That maps a human to the org to the CLIA to the ELR Party ID to the… and on. Sort of like:

Person/user
PA-NEDSS Disease Reporter Account
Trading Partner Profile
ELR Party ID
ELR upload permission

Hi Riki,
I also have a 'categorization' scheme because ELC Baseline wants this also. CT requires CLIA waivers for any location performing testing, so we should have CLIA codes for all testing locations. We ask for NPI but it may not be sent by non-labs. We use CLIA to track organizations reporting lab/test results in our processing and surveillance system. I've been talking separately with CELR and the Databot folks as we are sending to CELR using csv not HL7 and is the information needed to ID locations by CLIA going to CDC in CELR (we've changed our CELR pull recently so need to be sure)?

BTW - And I am being asked to now update testing venues into that CI/CT document by our state epi who is the ELC grant PI. :p

My current high level roll up categories are:
POC (non-hospital testing or other locations not included in other categories, includes FQHC and CHC)
Pharmacy (includes mobile for now)
LTCF-ALF-SN (called LTCF - includes home health employee testing)
Schools K-12 (including private schools doing a year 13)
Colleges (post-secondary)
Hospitals (including specialty hospitals)
Labs (commercial clinical - general catch all for labs vs POC or other categories)
Military (VA, US Coast Guard, Natl Guard - Army and Air, DoD)
CT Agencies (Dept Corr, and if a LHD has a lab its put here)

We can ID out rehab, home health etc. as needed.

We can share CLIA lists but would rather see CLIA go into CELR correctly.
