CVS COVID19 testing

Hi,
I may have missed this in another thread, but has anyone encountered the roll out of the Abbott NOW test to CVS or other similar locations? How have you set up getting results to the State or County HD?

thanks!

For a chain of urgent cares in TN they are using Abbott and faxing positives and emailing aggregate negatives. I received my first aggregate report today. They are also in the process of on boarding for morbidity reporting in NBS for positives.

I had an exchange with Jason Hall yesterday afternoon.  He and Jasmine Chaitram have met earlier this week and are meeting with Abbott today.... I had asked the following questions (after a bit of an intro):

1. Does CDC know how the COVID-19 testing materials are getting distributed? If yes, can that information be shared with state PH jurisdictions?

2. Does CDC know if the Abbot platform has any type of interface with a PC?  The LeadCare II machine does have an interface that creates an XML message; this interface is used in some facilities.

3. If "No" to #2, does CDC know if there are any plans to quickly develop an interface - either at Abbot or with EPIC, Cerner, and other EHR vendors?

We are preparing to use an Excel template for reporting POC COVID-19 results, but in order for it to work, we need to initiate - from Rhapsody - a secure e-mail to a known list of recipients.

FYI, Jasmine Chaitram and I have been talking with Abbott to discuss what's possible.  We have a follow up with them at 5pmET today.  Happy to update everyone with what we learn.  I have a call with the National Association of Chain Drug Stores Monday.  Once again, trying to see what's possible and where CDC can assist everyone.  Not trying to step on state toes or interfere with any work any of you may be doing independently with these chains.  Am also interested if any states are currently working in this area too.

Hi, one of our hospitals is using this in their ED and that is coming in the ELR feeds. I just got a text myself from an urgent care site I had visited that they are offering testing also.

Jason -- any help y'all can give is good help! It looks like it will be more than just drug stores, though.

We would want identified positive results at least. And aggregate negatives would probably work as well.

Let us know what we could do to help as well :-)

Can we request a standardized CSV format with core demographics, test, result and specimen data? I can’t see why states couldn’t import that data and most entities should be able to fill out an EXCEL file. Secure send… up to the states to work out delivery? AIMS? Other ideas?

I think that could make a lot of sense.  We're working with Riki at APHL on producing something that could serve this purpose.  I know she's working with input from states/CSTE too.

Amen Mark! we can get flat files in a secure email at the very least...

Alaska is building this process. We worked with our NBS host vendor InductiveHealth to build a route to convert CSVs into a format consumable by NBS. We’re providing the excel template to facilities in-state that have the point-of-care testing capacity and requesting daily files via a secure file transfer, that we can then import directly to NBS. I’m happy to share our tools with the group, and InductiveHealth is as well. Please go ahead and email me though at megan.tompkins@alaska.gov<mailto:megan.tompkins@alaska.gov> because my state IT blocks the basecamp site and I can’t access it directly.

Thanks for the info Megan.  There are actually a quite a few states that have built and are using CSVs that get transformed to auto import into their system, many out of necessity right now.  Some states can load directly, no transform, but having a format all states could use may still be worthwhile, especially for those states not yet doing this.  Additionally, some states may need assistance with the processing to get going, if their vendor or staff can't quite get it done for some reason.  On the last ELR call, Riki gave an update on the status.  I think she's made considerable progress since then.

So, we adopted the Oregon guidance when it came out on this site.  Let us hope that the laboratories will actually adopt a standard HL7 2.5.1 messaging process rather than trying to get each state a different csv file, or each state having to adopt multiple .csv to HL7 methods depending on the lab.

I just ask that if you are a state that is first out of the box and actually onboarded with any national lab with any one of these methods, please share it.

Right?

All,
I am meeting with CSTE representatives to discuss how to best create this national csv file format on Monday - I have used all the csv file formats that have been posted to this basecamp thread "COVID-19 Laboratories in other states" and am woirking on creating a "MEGA" csv file that covers all of these; and then create a route that converts to HL7 v2.5.1 ELR R1 message (either something that can be shared in diffeerent integration engine formats, or as a service on AIMS - just thinking out loud here).
If any other jurisdictions have csv file formats they want considered, please email to me at riki.merrick@aphl.org (mailto:riki.merrick@aphl.org)

Specifically for COVID-19 reporting to CDC I have created a csv file of a subset of those data elements - I am sure that will be shared shortly from Jason.

So hopefully be the end of next week I can have something out to you all to "throw mud at"

Hi all - I will hopefully be on a call with CVS reps today about reporting. They are standing up their first Minute Clinic with rapid testing on Friday! I'll share back what I find out. I need a clone, LOL

Riki Merrick, Terminologist at APHL Riki , I heard you on the ELDRS call this week. Is the csv file to HL7 2.5.1 a reality? How can we find out more?  thanks
Riki Merrick, Terminologist at APHL
once we have the starting csv file format, we are planning on adding conversion to HL7 v2.5.1 ELR R1 - just not sure if as a service on AIMS, or as routes for the different integration engines in use at the PHAs - open to what folks think is best

Rikki,

For PA either 2.5.1 or CSV will work. We use Rhapsody and the 2.5.1 would save us from having to mappings on our side.

Thanks!!

Rikki,

When you say a "service on AIMS," are you thinking that the data would flow from a test site like CVS pharmacy through AIMS to PH jurisdictions?
Todd Davis
Rikki,

We would prefer to get an HL7 2.5.1 file from AIMS.  We are a state that does not use Rhapsody for our ELR processing and would have to build in a work around through Rhapsody to get the data to our ELR translator.  We are live with Quest through AIMS and would really like to see these CSV's go that way as well.

Thanks

Yes that is one option we are considering - rather than having to implement the converter at each PHL,

Riki
Nancy Barrett, Epi 4/PH Informatics Specialist
Riki that would be great for us as well as we also do not use Rhapsody.
David DiCesare
We would prefer the HL7 in NY but can handle csv if needed.
