😢 Dual ELR Reporting Question

Minnesota reporting rule indicates dual reporting for laboratories (ELR) so that both the reference lab and the original lab/clinic should report the findings. We have spoken with a handful of facilities that indicate their laboratory information system (LIS) does not have a good way of identifying which reference lab results should be sent onto the health department (without major time/resource commitment to build something out using a data integration engine like Cloverleaf/Rhapsody). I'm looking for some insight on the following:
1.       Does your state require dual laboratory reporting?
2.       Are you currently able to get reference lab results sent from the ordering facility through ELR? 
a.       If yes, what LIS system(s) seem to be able to do this or is a data integration engine being used?
b.       If no, which LIS system(s) seem to have the most struggle with this?
3.       Does size of the health system or facility affect dual reporting?
4.       Does your state have health information exchange(s)? Does this make a difference in receiving reference lab results from the ordering facility?

1.       Does your state require dual laboratory reporting? No, Iowa implemented an exemption in 2013 for ordering facilities as long as the performing lab reported on their behalf.
2.       Are you currently able to get reference lab results sent from the ordering facility through ELR? I believe there is only 1 facility that dual reports in Iowa.
a.       If yes, what LIS system(s) seem to be able to do this or is a data integration engine being used? Sunquest LIS with an EPIC EHR.
b.       If no, which LIS system(s) seem to have the most struggle with this?N/A
3.       Does size of the health system or facility affect dual reporting?N/A
4.       Does your state have health information exchange(s)? Does this make a difference in receiving reference lab results from the ordering facility? Iowa does have an HIE, but this is not a factor.

1.       Does your state require dual laboratory reporting?
Dual for us means 2 things -- reporting to local health and the state, and reporting of reference results. Our current regulations require the submitting lab to send results, therefore hospitals are asked to send referred results. We do make some exceptions, such as for blood lead. Since we are not 100% complete with ELR, we will need to re-assess later in 2019, but we do know that reporting of ordering provider is incomplete from referred results.
2.       Are you currently able to get reference lab results sent from the ordering facility through ELR? 
Yes with our limited experience, but we have had at least one hospital working with us for ELR onboarding tell us they will need to put in special work to do this.
a.       If yes, what LIS system(s) seem to be able to do this or is a data integration engine being used?
I've only heard that this is something from the LIS that is needed.
b.       If no, which LIS system(s) seem to have the most struggle with this?
I think it was an EPIC hospital, but don't quote me. :-)
3.       Does size of the health system or facility affect dual reporting?
not for our requirements
4.       Does your state have health information exchange(s)? Does this make a difference in receiving reference lab results from the ordering facility?
HAHAHA, No. We are in our fourth attempt to get an HIE, so we are not holding our breath on this one.

2.       Are you currently able to get reference lab results sent from the ordering facility through ELR? Yes
a.       If yes, what LIS system(s) seem to be able to do this or is a data integration engine being used? The health systems that are capable of doing this in TN use Cerner. Sometimes the results being sent from the reference lab are not formatted according to standard and they fail in our surveillance systems. In the case, Cerner has to make modifications on their end to change some of the content of the message in order to correct the error. For example, the reference lab coded OBX-2 as ‘CE’ or ‘SN’ but OBX-5 is string. Cerner will change OBX-2 to ‘ST’ in order for our surveillance system to consume the message. We also have them add a NTE indicating they are reporting the result for the reference lab so it is clear when the reference lab also reports the same result.
b.       If no, which LIS system(s) seem to have the most struggle with this? N/A
3.       Does size of the health system or facility affect dual reporting? No
4.       Does your state have health information exchange(s)? Does this make a difference in receiving reference lab results from the ordering facility? We have a HIE, but it is not used for ELR data exchange.
