ICD-10 codes in ELR

Hello, 

Our HIV surveillance program asked us recently about the use of ICD-10 codes in ELR messages. After some digging, it appears about 10% of our messages include ICD-10 codes in OBR-31 (Reason for Study). The program wanted to know if we could require facilities to report this information in ELR messages for the purpose of prioritizing results that include an HIV diagnosis code. Because all CD4 and HIV viral load results are reportable in TN (like in many other states), we end up with a decent number of results for individuals that are not actually HIV positive. As a result, the program is exploring ways to more efficiently process lab results. I was curious if other sites have experience using diagnosis codes in OBR-31 for processing/prioritizing ELR results or if there were other ways this information has been utilized.

We haven’t used these for this purpose, but are in the process of developing a method to pull pregnancy related ICD-10 codes to flag patients who may be pregnant for additional follow-up.
