Interpartner Connection Out of State Labs

Hi, MT is wondering what states do with out of state labs once an interpartner connection is complete. MT currently sends the labs to the other state as well as into our own surveillance system. We are looking to not send it to our own surveillance system, but wanting to get an idea of what other states do in this situation.

Jennifer – In Idaho, we try not to keep non-resident ELRs in our surveillance system. Legal opinion is that if they aren’t an Idaho resident, we don’t necessarily have authority to retain their PHI in our system for sporadic (non-cluster or outbreak) cases we don’t investigate or intervene on.

Iowa validates Patient, Ordering Provider, and Ordering Facility addresses on all in-bound ELR right away, then evaluates by disease (there are some additional rules related to STI and HIV) and then refers to the other jurisdiction, if appropriate. For most infectious diseases, routing to the other jurisdiction is based solely on patient address. For STIs and HIV, there are 3 possibilities: Refer to another jurisdiction, Keep (send into Iowa systems), or both. Iowa doesn't retain anything but a little metadata in Rhapsody for all records referred to another jurisdiction and not kept.

We are sending COVID, Gonorrhea and Chlamydia bidirectional ELR data with other states. If the patient is below 18 or above the age of 25 we automatically forward to the other jurisdiction and import the file into our system.
If they are between 18 and 25, we assess the true residence of the patient and handle it from there. We do some minimal logging on all messages to assess the inbound and  destination(s) it might have taken, i.e. import only, import and sent to the other jurisdiction.
COVID results are all automatically forwarded and not imported.
