Monkeypox Testing LOINC-SNOMED codes
Good morning,
I've emailed Riki Merrick about any codes for Monkeypox testing, but thought I'd post here also. In CT, we are going to request labs report these results
I've emailed Riki Merrick about any codes for Monkeypox testing, but thought I'd post here also. In CT, we are going to request labs report these results
- Detection of monkeypox virus (MPXV) or orthopoxvirus DNA by PCR testing in a clinical specimen
- Detection of presence of orthopoxvirus by immunochemistry in tissue
- Detection of MPXV or orthopoxvirus by genomic sequencing in a clinical specimen
- Detection of anti-orthopoxvirus IgM antibody using a validated assay on a serum sample.
Currently in LOINC there is the following (as I'm sure others have seen)
| 10700-3 | Orthopoxvirus [Identifier] in Skin by Electron microscopy | Orthopoxvirus | Prid | Pt | Skin | Nom | Microscopy.electron
41853-3 | Orthopoxvirus DNA [Presence] in Specimen by NAA with probe detection | Orthopoxvirus DNA | PrThr | Pt | XXX | Ord | Probe.amp.tar
SNOMED code 59774002 | Monkeypox virus (organism) |
Thanks all
I have been working with CDC on the LOINC/SNOMED CTs for the PCR based test - here they all are:
Initial Test:
Can you share the SOPs or package inserts for the other targeted test, so I can finish putting together a guidance document with all the monkeypox related codes?
Riki
100434-0 Orthopoxvirus.non-variola DNA [Presence] in Specimen by NAA with probe detection
I am still looking for the SOPs for the other test types you are mentioning :)

Let us know when these codes based on the guidance are Ready!
Nancy
Do you have any SOPs for the other test types?
With the focus on LOINC and SNOMED codes are you expecting that Monkeypox test results will be sent in HL7 format via the PHLIP interface?
Thank You

There was some discussion on the Open ELR call about not including positive / negative in the value set (as that is what is used in the package insert - however detected not detected is the preferred terminology for PCR resulting and in the SARS-CoV-2 LIVD file all positive is mapped to detected and negative is mapped to not detected) - they may update / add some guidance around that.
I will be submitting 10 PLT codes for tests performed at the CDC which are more specific target PCRs as well as Elisa for Orthopox IgG and IgM next week, hopefully in time for the next release in July.
There is a difference between the codes used in this document (LN Code) and the LRN codes for non-variola orthopox PCR test (99LRNBTST code). Having 2 different codes for the same test is problematic, do you know if the LRN codes will be updated to this new LOINC code?
Thank You
Robb
I am no longer working on LRN vocab, but I checked and the LRN messaging guide will be updated today to include the published LOINC code. If you want to update your LRN message to use the published LOINC code, LRN will be able to support its use.
If you are worried about the LOINC being a pre-release code - LOINC release shoudl be coming out officially in the next 2 weeks I think, if that affects your timing.
Note: I am opposed to adding these to CELR, but adding to GenV2 is feasible.
Thanks!
Thank You
