Nursing Home testing - getting results

HI all,

Does anyone have experience with a nursing home getting the BD Veritor or Quidel Sofia antigen test instruments results? What have you done? Assistance that nursing homes needed? Barriers, issues?

BD Veritor has a USB port where it can be linked to a computer and a csv file sent. 
Quidel Sofia has a USB port but data can be put on a 'stick'/thumb drive.

Can you post that here?

Nancy, I have not seen anything like this. Most LTCFs are attempting to report via manual reporting/fax which is extremely burdensome for them and for us. 
Do you know both of those instruments generate a CSV that can be utilized by the APHL tool? Or do they have to modify the CSV to meet different states flat file versions?

If we know the format of the csv file we could figure out a way to populate the HL7 generator csv file for processing and use that?

I was trying to ask a couple of questions on the CELR call and I had a double mute situation and couldn’t get both undone quick enough so it has been one of those weeks.

So, we don’t want to have to add a thousand CLIA numbers to our systems unless we know they will have data for us. Do all these facilities have a way to notify you or us that will have data for us or are you just expecting us to turn it on for all of these? I cannot see Maine or North Dakota having data for Illinois from all those Abbott now labs so we have no reason to add those CLIA numbers to our system.

We also wanted to know about the flags as we are still getting AOE questions on the Quest Chantilly data and it is blowing up. We have to go through the file and delete all those segments to get those files to process. Do we have our flags set correctly or is the Chantilly file separate?

Since you do have these flags does that relieve us of the all or nothing for turning on the AOE? We are trying to upgrade all our servers, databases and other infrastructure changes to try and keep handling all this data coming in and unfortunately this is all falling on the same 3 or 4 people as everything else. So, if we can move that to the side for now that would be great.

Do you have an example of a CLIP number? I tried to google that and I am not finding what it is or how it is formatted. We would blow up if “CLIP” was sent as the MSH 4.3 field as we have a cross reference table on that field and CLIP is not in that table. We may need to add some code if that would come in.

Hi Tood,
For the questions I can answer:
#1: I have notified Quest to stop Chantilly from sending AOEs - and they said they told them to stop = sorry that has not kicked in yet.
At the moment the flags are implemented on the CLER route, but I just opened a ticket to see if that can be copied over to the quest route - stay tuned on that front.

#2: example CLIP# for FT BELVOIR COMMUNITY HOSPITAL = DOD2206001; I have also assigned them an OID = 2.16.840.1.113883.3.8589.4.2.12.1 and a namespace = FTBELVOIRCOMMHOSP

I don't know if all these facilities would have results for IL, when they are not located in IL, but theoretically I think they could since reporting is based on patient resident state. Thinking out loud, you could log all the CLIAs for IL and neighboring states and not log the others - if you get a CLIA you don't expect, them you could go looking to see, if they are listed on that smartsheet?

I just wanted to mention that in MD we have started to get data from DOD facilities and we are seeing addresses all over the country (and world) for people. That is certainly not every facility but it makes me wonder if they are applying the same residency rules we would to ID.

I am asking myself the same question - how do we handle non-US patients, are we ok to send to the PHA of the testing lab?
