Patient Visit group in ELR: Feedback requested

During the HL7 Public Health workgroup call today, a JIRA ticket regarding PV1 in ELR was discussed. The link to the ticket is https://jira.hl7.org/browse/V2-25344

The original ticket comment asked “In LRI_PH the visit group is RE, but in LOI it is O for any profile, except the LOI_FI. Shouldn't LOI_PH have usage of RE for visit group, if it is RE in LRI_PH?”

In the call discussion, there was interest in seeking clarification on “whether the LRI_PH conformance statement to have visit group as RE is intended to apply to all jurisdictions, or whether it should actually just be O since it only applies to some jurisdictions.”

The question we are trying to answer is: Do we need to consider constraining the patient visit group in LOI PH profile from O to RE, or if we should relax the LRI PH profile from RE to O for the visit group.

Please reply to this thread by Friday, May 6 to indicate if your jurisdiction is needing/expecting the patient visit group (namely PV1) in an ELR message.

Ohio generally requests PV1/2 segments to be suppressed but if they are sent then PV1-2 accepted values only include E, I, or O.

IA can accept the PV1 segment, but does not require them. We can process the message, if it has PV1 segment.

MN can accept PV1 segments, but doesn't require them. Similar to Wisconsin, we will extract hospitalization information into our surveillance system when provided. This has been helpful for some reportable conditions that are only reportable in the event of a death or hospitalization.

Idaho can accept but does not require PV1.  If a PV1 segment is in the message then PV1.2 (Patient Class) is required and can be one of the following E/I/O.  This information is used to indicate hospitalization status.

Washington state ELR does not require a PV1 segment from submitters, however we can accept these segments when they are sent. A subset of our submitters currently send these segments to us without issue (consistent with the ELR 2.5.1 IG)

Virginia's IG states the PV1 segment is Optional, but implementation guidance is included because some programs use the information when it is provided. The segment is not mapped in our NBS system.

Clarify on PA: we do not reject messages that contain PV segments and do not validate the segments during onboarding. We ignore the segments but could add them to our parsing logic if directed to do so by program staff.

Sorry for delay...OOO last week.  Kansas does receive and process the PV1 segment from several reporters for hospitalization data but it is not a state requirement.  Thanks.
