Providing a list of sites submitting for CLIA certification

Hey all,

Hoping to get additional offline feedback on the topic raised on the 11/10 ELR call regarding providing information on sites submitting for CLIA certification. 

CSTE is working with APHL and CMS to be able to provide an updated, comprehensive list of sites that are applying for CLIA certificates of waiver, compliance, accreditation, and PPM. This list could serve as additional resource to reference if there are gaps or lags in state-maintained lists. 

First question - Would this resource be useful for your jurisdiction?

Second question - If yes, which info would you want to know about those applying sites? See below for the data fields that are currently possible to provide:

•CCN
•Date CLIA # was assigned 
•Certificate status
•Current application code
•Current certificate code
•Current effective date
•Current expiration date
•Pending application type (when no current is present)
•Pending certificate type (when no current is present)
•Current lab classification code
•Pending lab classification code
•Facility name
•Facility street address
•City
•State
•Zip code
•Zip code (+4)
•Phone number
•Lab type
•Facility email

I would appreciate any insight on the utility of this potential resource.

I think adding the OID for the organization would be good - this might involve PHINTECH for those that have NOT YET been assigned any, which will take some time, but for those that are in the system it would be good to have.
Both facility OIDs and application OIDs for their data producing systems (LIMS/LIS)
