Question on use of PHINMS vs other secure messaging platforms

Hi all,
We are trying to move our data tiers to 2019 SQL servers at our state data centers but PHINMS is not compatible with that. CDC so far says we are the first state that has brought this up.

What are other states thinking of for moving forward?
Keep PHINMS
Migrate to using AWS S3
Other?

Please let me know. We can also ask this on the weekly ELR call or workgroup.

I can tell you we (Oregon) struggled with PHINMS and our most recent attempts at upgrade failed horribly. We switched to S3 for all but 3 direct send facilities last year and abandoned it altogether a couple of months ago. We worked with PHIN Tech to try to resolve issues for a couple of months before throwing in the towel.

Hi Nancy,

Idaho is trying to move away from PHINMS as well, we have eliminated most of the use of PHINMS but still have a couple of scenarios to change. We have moved to the following:

Incoming ELR (Labcorp, Mayo, etc.) - SFTP (Currently in-house SFTP server but we are looking at S3 SFTP)
Incoming ELR (Quest, Cancer, etc.) - AIMS S3 Interface
Interpartner (including PHLIP, Rabies and LRN reporting to CDC) – S3 via the AIMS Hub
HL7 Notifications – CDC’s S3 SFTP interface

The AIMS folks have an encryption filter for Rhapsody/Mirth that will encrypt messages using certificates so that they look like the PHINMS encrypted messages. We use that for the CDC PHLIP and LRN messages via AIMS.

Hi Michelle, my data exchange tech person has done a lot with PHINMS and CDC so let me connect you in case he can help.

Rob - we are using AWS S3 with AIMS for Quest (elr and cancer), interpartner, ecr as well. I'm not sure about LRN. Thanks for the note about STFP S3 as that may be an option.

Did you run into any particular issues moving hospital labs to S3?

Thanks Nancy, but we've given up on it. :) We switched our direct send (Mayo, LabCorp, and Planned Parenthood) to SFTP. We switched our our syndromic to SFTP too. Everything else is going through AIMS S3 (both incoming and outgoing).

The hospital labs are all sending via SFTP, we are setting up a proof of concept using S3 SFTP but that will still just look like a SFTP client to them.

The CDC SFTP S3 interface for HL7 notifications is awesome, it is so much better than PHINMS. The speed is outstanding, when onboarding HL7 COVID MMG and CDC wanted a back-load we were able to complete it in 2 weekends. PHINMS would still be working on it now 😊

Michelle, how do you send ARBO notifications to CDC? Currently we still use PHINMS for those as they cannot go via the SFTP S3 interface (for now, it is apparently being worked on)
