Tesfaye and Svetlana,

I apologize for updating earlier our CASPER document with the wrong title. This new uploaded file today is named more appropriately (I included "master document" to the title, per your suggestion.)  The ONLY content change was deleting the Texas Dept of State  Health Service website url, per their recommendation. Evidently their url is in transition. thanks again.  Dave
