A huge 'Thank You' to the Evaluation Workgroup for development of a "Survey Bank" resource!  Please read the information from the workgroup carefully below.  Questions related to this resource can be directed to Lauren Polansky (hhw3@cdc.gov).

Instructions/Purpose of Survey Guide:
  • The survey bank is just that…a “bank”. We borrowed each of the questions from existing surveys made by the states. Our thoughts were that the "bank”  would be the first step in thinking through and selecting common questions to cover the basic evaluation areas (background/personal experience, knowledge/awareness, attitude/motivation/values, and intention to change).  Our hope is that it will help anchor us all through general topical areas (zoonotic disease, agritourism, general influenza prevention and swine exhibit influenza prevention) that we know people want to learn about and impact through the educational activities/interventions that they’ve designed. It is not meant to be exhaustive. Just thorough enough to account for a basic coverage of desired information. 
  • We strongly suggest making sure that each of the evaluation areas (background/personal experience, knowledge/awareness, attitude/motivation/values, and intention to change) are covered with a question.
  • If a question is selected by a state, it can be adapted to match that state’s specific population, environment or activity being evaluated by inserting/replacing a word or two (examples could be zoonotic disease rather than influenza or farm animals rather than show animals, etc). We tried to use this approach as best we could so that we wouldn’t wind up repeating the same question for multiple populations or environments., which is a never ending road!  We’ll have an opportunity to work with states to review any survey that is made, which would be a good time to edit for a specific intervention or population.
  • The surveys are targeting youth. So simplifying the language to be as direct as possible is essential, while making sure that we are still communicating the main intent/message. Thanks so much to everyone for their additions and suggestions in this area…all the help is much appreciated.
