Summary of AR Required Reporting, CRE Definitions, and AR Activities in New England

Connecticut
Members: Richard Melchreit & Lauren Backman
AR Reportables: CRE, Carbapenem Resistant Acinetobacter baumannii (CRAB), MRSA (technically all invasive SA regardless of AR status) & C. auris (technically all regardless of AR status)
CRE Definition Criteria: 
  • CDC definition (Including all Enterobacteriaceae)
  • Isolates that test CP positive locally are reportable to regardless of resistance profile
  • Cases are reportable once every 30 days, unless additional genus/species or worsening carbapenem resistance profile is observed
  • Reportable from all sterile sites, sputum, and urine, but not stool
  • CP positive screening isolates are reportable   
High Level Description of AR Activities 
  • All CRE/CRAB reportable with required Isolate submission
  • Antibiograms are requested at time of report
  • CRE isolates undergo MIC testing (Sensititre, Kirby Bauer), mCIM, and carbapenemase PCR testing at state public health lab
  • CRAB isolates are forwarded to ARLN regional lab for testing
  • All non-KPC CP-CRE, and all CP-CRAB receive “timely” investigation including surveillance swab collection of healthcare contacts
  • All CRE/CRAB have case follow-up per EIP MuGSI
  • All MRSA have case follow-up per EIP invasive SA protocol
  • All suspect C. auris that cannot be ruled out at local clinical labs should be submitted to SPHL for confirmatory testing
Maine

Massachusetts

High Level Description of AR Activities
  • Production of statewide antibiogram (annually) using antibiograms from all acute care facilities in the state
  • ​​Nine-month educational program for LTCFs on AS and infection control (monthly webinars, calls and antibiotic start data collection) in collaboration with QIN-QIO
  • Exploring alternative ways (other than NHSN) to collect AU data from acute care facilities-may adopt an online survey tool to collect data
  • Investigation and control of all identified cases of CP-CRE  with novel resistance mechanisms identified, C. auris and mcr, including contact-tracing and PPS where indicated
  • Facilitate statewide combined HAI/AR technical advisory group
  • "Bug of the Month" webinar series for broad audience including labs, IPs, LTCF, etc. including presentations on C. auris, CDI, CP-CRE, mcr.
New Hampshire

High Level Description of AR Activities
  • Facilitate monthly meetings with our Antimicrobial Resistance Advisory Workgroup (ARAW) made up of subject matter experts and antimicrobial stewardship leaders throughout the state.
  • Publish a statewide antibiogram annually including acute care hospitals and reference laboratory data.
  • Hosting an Antimicrobial Stewardship Symposium, May 23rd 2018.
  • Work closely with the Public Health laboratory and area ICPs to investigate reported cases of CRE and CP-CRE. 
  • Utilizing social media as a method of outreach to the public regarding antimicrobial resistance as well as antimicrobial stewardship. 
  • Developing statewide recommendations regarding CRE.
