Feedback: I wish that the skip patterns didn't make you click to the next page because I think it adds confusion. I liked that the Wufoo form was one single page that, while long, you could still see the end and all of your options at once. If we use Qualtrics, I wonder if it would be better to have each type of modifier start on a separate page - if I scrolled too quickly, I missed the header that separated modifier sections. We could list the types of modifiers at the beginning so they know what is coming. 

I also wish you could go back after a skip patterned question - I know this is a limitation of Qualtrics, but I'm concerned if someone mis-chose an option (like age group vs. age range), they will try to go back, then exit out and reopen the link (which of course just opens back to the page they were last on). 

Age Range is missing a sub-question: Units of Time (Hour(s), Days(s), Week(s), Month(s), Year(s).

Antibiotic/Antibiotic Sensitivity is missing a skip pattern - if selected any antibiotic, the user was forced to answer this sub question:

Also, no matter what we use for the new condition subset, can we add a note that says: Please choose condition modifiers based on what is written in your jurisdiction laws/rules/regulations. Unless otherwise specified, the chosen modifiers will be included in the new condition/subset name.
