Thanks Natalie - you bet me to posting. Appreciate adding this and would love to see others that you might be using!

I've asked for ours but in the meantime one comment I have about checklists.. is the constant need to remind people to go back to the checklist. I get a ton of requests for "how do I get access to X system" and I'm like "wait, I know that's on the onboarding checklist. Just look there before you ask me" so once it's compiled, make sure that people know where to find the most current version to use it! Sometimes people also don't know they need access to SAS or GIS or whatever it is right away, so they've been here a few months and THEN they try to get access to things. And by then onboarding is long gone from their brain so they don't think to look there for info on how to get access to whatever they need.

Great point Erica - in my survey about this staff suggested "how to" guides which I am thinking might really just be orientation material re-branded - I am going to try to stand up a SharePoint site for all this to be housed so it's easy to point back to. (or at least that's the plan)

Here is the onboarding checklist form we use for all new hires in Chicago. It is pretty basic and does not really cover getting access to role-specific software (e.g., SAS)

Katrina Hansen Katrina  (I think), was it you that mentioned a role-specific form? Would you mind sharing your epi onboarding form?

Thanks Kelly - I like the category breakdown in this one

Hi all, I searched but our prior Division onboarding checklist disappeared! I think they are updating it now since a lot of policies have changed. Some key elements that I remember it included: where to find various Division wide policies/ procedures (admin operations, travel policies, HR, etc), details on where to find things on our website, key contacts for the Division and organization, and lastly had a meeting at the end to meet with each Bureau Chief. It reminded me a lot of the one shared by CO.  I have created very simple checklists that are job specific and we have some COVID specific ones. I will see what I can find that would be useful for this broader group.
