For laboratories that are offering at-home sample collection, what information are other states requesting for ORC-21 through ORC-23 (Ordering Facility Name, Address, and Phone)?

We require the same information as for any other test performed at a laboratory. However, for these tests, that information may not be available - I don't know what is required to request a test. So if not, we'd need really detailed patient demographic info, including phone numbers for follow up.

Thanks Nancy - so if the test is initiated by a patient, collected by the patient and then sent to the lab, do you ask the lab to put the patient's information in the Ordering Facility fields? Or would you have them send something like "at home test", or the preforming lab, or something else?

In Maryland, we have not used the patient name. We have used something like "Self Refer" for the provider name or ordering facility.
