The purpose of the eCR/RCKMS Community of Practice Basecamp is to provide staff from state, local, territorial, and tribal public health agencies with a venue to participate in peer-to-peer discussions pertaining to using RCKMS. This Basecamp is not intended for formal communications about RCKMS or eCR and will not be moderated by CSTE staff. ✨ RCKMS Content Team Seeking Feedback on Birth Defects

As the RCKMS Content Team is planning for future releases (2023 and beyond), we are looking for jurisdiction input on the following:
  1. Would any jurisdictions be interested in receiving birth defects reported through RCKMS (case data based on the eICR implementation guide, not the birth defect implementation guide)? 
  2. If yes, would you prefer to see individual Reporting Specifications for each condition or a consolidated Reporting Specification that covers all birth defects (similar to how RCKMS has implemented Cancer)?

If there’s capacity, I think there may also be interest in notification of a birth event. That would help drive blood spot testing by serving as a separate notification that the newborn testing lab should be expecting a sample and associated test request. A birth certificate isn’t as helpful for a notification because of allowable delays in reporting, but the newborn testing isn’t dependent on the infant’s name.

Let me discuss the birth defects piece with our internal team- we currently perform active surveillance, reviewing records. One concern about utilizing RCKMS is the consistency of data and how much information reported is extracted from/reliant on progress note information and not solely on codified fields.

Seeking clarification on behalf of our STD Branch... Would the birth defect trigger occur IF a child was a stillbirth / adverse congenital syphilis outcome at birth? If so, I think we would be interested in it.

Hi Melanie,

This would depend on how a stillbirth is coded within the EHR. If that is entered as a diagnosis for example, we could potentially implement a Stillbirth Reporting Specification that would capture these events. We can note Stillbirth as an event of interest for Birth Defects. For Congenital Syphilis specifically, we do have a Congenital Syphilis Reporting Specification that is currently available for authoring within RCKMS. Please let us know if you have any questions about implementing that condition.

Currently the RCKMS Content Team is only discussing Birth Defects, but we will note the interest in notification of any birth event. To clarify, are all birth events reportable in TX? Thank you for your input and for discussing birth defects internally!

Minnesota would have interest in adding Birth Defects to RCKMS, but concerns were brought up around the difference in definition of Birth Defects used at different jurisdictions (so would be curious as to what trigger codes and the broadness of them to be used).
We are still mulling over the implications of different conditions vs all one condition as to what might be best.
