ABOUT THIS COMMUNITY OF PRACTICE In partnership with the National Center for Injury Prevention and Control (NCIPC), the Council of State and Territorial Epidemiologists (CSTE) hosts this community of practice among OD2A-funded jurisdictions currently conducting innovative surveillance projects. This OD2A Innovative Surveillance Community of Practice consists of 3 focused workgroups that provide a forum for peer discussion on cross-cutting topics, methods, and challenges/solutions across jurisdictions. ______________________________________________________________________________________________________________________ USING THIS BASECAMP + This Basecamp is a tool that houses previous call slides/recordings and allows members to engage with peers. + The Q/A section may be used to facilitate connections/discussion among members. Engagement could include posting new questions/discussion topics or replying to peer questions/discussions. + Members are encouraged to share helpful resources for peers. Members may post any publicly available resource in this space. Indicators Project | Data Flow Diagram Debrief

  1. How does you program define linkage to care? 
  2. Does your program track referral and linkage to social, behavioral, or other support services, such as housing, employment, or harm reduction services? 
  3. What activity or transaction from your program is missing from the Substance Use Disorder Continuum of Care data flow diagram?
  4. Do you have any documentation that you can share that describes the data elements shared between systems or details of that sharing (e.g., manual vs. some automation, use of any applicable standards, etc.)?

1. Linkage to Care for us is ensuring that a patient receive the appropriate level of care throughout their treatment.
2. Yes we track referrals to social, behavioral and other support services through Openbeds.
3. We struggle to get up dated info on individuals going through the legal process or if they have additional medical conditions.
4. We create a monthly document with the referrals that have been created. Also, we have been able to collect the number of referrals that mothers/infants on a CARA plan need. We do have some processes that require manual vs. automation.
