Memorandum of Understanding (MOU) Examples

If there are any sites that would be willing to share examples of MOU’s they have used in the process of recruiting providers for this project, please attach them to this message board thread. We would like to be able to share them with other sites that may still be in the process of recruiting providers as we believe it would be helpful for them to see. I suspect we are of little use here. We are operating out of the same 5 clinical sites that we have engaged since 2009. Our dispersed sites are all within one health system and are part of the University of Wisconsin Department of Family and Community Health. Accordingly, we never required a written MOU.

On the other hand, we have had a very mutually beneficial relationship though the provision of RIDT kits, comprehensive multiplex testing and ongoing, weekly feedback and education.

I appreciate the opportunity involving in the process, thank you.

I feel there is no collection instruction for practice site, I provide this information for you.


* Swab specimens should only be collected on swabs with a synthetic tip (such as polyester or Dacron) and an aluminum or plastic shaft. Swabs made of calcium alginate or that have cotton tips or wooden shafts are not acceptable.


* Specimens should be collected using standard technique and placed in viral transport medium. A minimum of 200L volume is required. Specimens can be stored for up to 72 hours at 2-8○C or at ≤-70○C for long term storage.
