Joe & I have revised Chapter 2 extensively, cutting out quite a bit in an effort to adhere to what I think was a consensus opinion that the chapter ought to be much shorter, basically constituting an introduction to the 3rd edition of the Guidelines (it’ll actually become Chapter 1) and reserving most of the detail found in the current Chapter 2 for subsequent topic-specific chapters.
 
At this point we really need some input on the overall gist of the chapter.
