Labcorp is receiving feedback during our 2.5.1 validation that some states cannot accept Local Codes in place of Order LOINCs in OBR 4.1.
We are unable to assign test specific LOINCs for some of our more complex panels or Superpanels. Is there a general LOINC that can be used in these cases that will be accepted by states?

Interested in how many states are actually having successful inter-partner transactions/onboardings without having to implement extensive logic to clean up ELRs to send/receive.

Is anyone else seeing an issue with the way Quest is reporting Tspot and IGRA? We have noticed there is a shared LOINC and the only thing that differentiates the Tspot is the local code (86024531 (T-SPOT.TB)). The LOINC we are seeing used is 71773-6

Hi - I was asked recently about ELR of genetic sequencing - has anyone done this and how? I know we get sequencing for HIV, but have not tried to figure out how to process it in our data exchange system for eHARs. thanks

One of the challenges we are faced with post pandemic is setting up quality assurance systems - the landscape has changed dramatically because of the sheer volume of data, and tools that we used pre-pandemic don't necessarily work as well anymore. With the custom rules Lyme MTTT, there appears to different test result rules per lab per LOINC. How are are other surveillance systems handling this request?

What is the precedent for labs sending pregnancy status for conditions that require pregnancy status (for eg. Hepatitis B, Hepatitis C, HIV)? Has any jurisdiction been successful in getting their labs to send pregnancy status other than AOE questions? Maybe through their IPs via paper?

HL7 needs help with defining the value sets for Gender Identity and Sexual Orientation that will work for ALL states - this is a rather urgent topic, as NU introduced legislation that requires this to be included in lab messages (orders and in ELR), and CA already requires that in ELR for COVID tests and I am sure there are more rules that I don't know about. While HL7 is working on where to include this in ANY HL7 v2 message (not just lab), we need help ensuring that ALL PHAs use the same vocabulary (maybe not all need to support all terms, but ALL terms need to be codified the same way). What are others doing with data coming in with Chinese characters, Spanish accents and now nicknames in quotes as part of the name? These will load using our interface engine however our Java Application is having issues with this data and we are spending a lot of time deleting this data or changing it so it will import correctly. Do others have issues with this?

Would it be possible to have a brief discussion on the collection of sex at birth vs gender from ELR. I'd like to hear about current practices concerning the collection (or not) and any concerns about the quality of sex at birth and/or gender sent via ELR, particularly related to HIV surveillance.

Is there a consensus among the states for AOE reporting in ELR in terms of once per COVID test, once per message, or perhaps only wanting it on direct detection (PCR, Antigen) and not on the Antibody tests?

Nancy and others - would you like to discuss (again) the issue of specimens transitioning through multiple labs and the problems that introduces regarding getting accurate and complete information on the originating treating health care facility for the patient and provider information? One thing I was wondering was whether we could explore what the steps would be to add or revise the ELR HL7 specification to enable capture of this information, and perhaps the entire "chain of custody". This seems important and I know it is a big effort and will likely take a long time to achieve but I was thinking we could discuss what the steps are and any obstacles - are there ways to get it started? Do others agree? Having the originating facility report is kind of a bandaid and not all facilities can really do it for all results. Thanks for your thoughts!

I would also like to add a topic to the call regarding reporting when multiple laboratories are involved. We have encountered some situations where a provider in our state orders testing from a laboratory and then that laboratory sends that order to a third laboratory. When laboratories send testing to a third lab, sometimes the patient address information is not provided by the intermediate laboratory. Since the ordering facility is then the intermediate lab rather than the original ordering provider, the results are then reported to the state where the intermediate lab is located (or if that state has different reporting laws, the results may not be reported at all). If the state where the laboratory is located does receive the results, it then falls on that state to follow-up and make sure the results are reported to the original state of origin. How do other jurisdictions handle reporting via pass through labs?

We (SD) have noticed that for a given patient’s ELR record, we could have multiple races reported for one ELR. This practice makes it difficult to perform effective EPI analysis on the data. I noticed that the syndromic surveillance HL7 ADT standard only allows for one race to be populated; the ELR standard allows for multiple races. I would like to hear from other jurisdictions how they are handling or plan to handle the multiple race issue, specifically for EPI analysis. In my opinion, the best way to address this issue is to modify the standard so that only one (primary) race is allowed in the ELR standard.

The Virginia Department of Health also would like to suggest a topic of discussion. We would like to know how other agencies using an NBS system are consuming Whole Genome Sequencing data, e.g Salmonella. Does anyone receive this data via ELR? If not, how are you receiving it? We are are not currently receiving WGS data via ELR, but would are looking at ways to do this.

Utah Department of Health would like to ask our national partners about how their jurisdiction processes specimen source when sent electronically. For example, do they enter in the specimen as it is sent in the HL7 message or do they standardize to a finite list of approved specimen sources in their surveillance system?
